Why Barcodes and QR Codes Fail to Scan

Scan failures usually come from inadequate printed size, missing clear space, weak contrast, poor source artwork, surface reflection, distortion or incorrect data. Test the code at final printed size with the actual scanners and phones that matter. Successful scanning on a large screen is not evidence that the production tag will work.

Diagnose the code systematically

Check the encoded content separately from the printed symbol. A visually neat code can still contain the wrong destination or identifier.

Begin with a controlled source

Use a standards-appropriate code generated from the final data rather than tracing or redrawing a screenshot. Keep vector artwork where possible and avoid effects, transparency or decorative modification. For variable records, validate the data file, record count and mapping between each code and product version.

Test the production-sized artwork

Print a one-to-one proof without letting software fit it to the page. Scan at different distances, angles and lighting conditions. Retail barcodes should be checked with the operational scanner, while QR codes should be tested on a reasonable range of phones and with the live destination available.

Protect the code through the specification

Keep the symbol away from drilled holes, folds, perforations, strong texture and highly reflective finishes. Recheck after any resizing or artwork revision. Record who validated the data and retain the approved version so later products do not inherit an obsolete price, SKU or web address.
